Instructions for Rating Your Abilities
In this survey, you will be asked to rate your level of functioning for a series of different abilities using a scale from 1 to 7, where 1 represents the lowest possible level and 7 is the highest. If you cannot do any level of an ability, please mark that option below the numeric scale.
When giving your rating, please follow these instructions:
Each ability will be presented with its name and definition. Please read the definition carefully before providing your rating.
Rate yourself based on your current level of ability, not what you were able to do in the past or what you could do in the future with additional training. If you use an assistive device (e.g., glasses), please rate your ability when using the device.
To clarify what we mean by ‘level’, we provide you with examples of tasks or activities at different levels for each ability. These are intended to help you determine where your ability fits on the scale. The activities might be from contexts that are unfamiliar to you; please focus on whether you can do the indicated activity or one of equivalent difficulty.
We will ask you to rate four different kinds of abilities: cognitive, psychomotor, physical, and sensory abilities. After you have given your ratings, we will ask you some follow-up questions.
